To receive a stipend from the C/SE ABLE Resource Center you must fill out the following forms: Stipend Request Form and Payment Compliance Form (this form only needs to be completed one time and will remain on file at Ohio University). Please return all forms to the C/SE ABLE Resource Center and kindly allow 6 weeks for processing.
